This is great. There were two things that struck me about the revelation in 'Birthmarks'. One was that House was no doubt a keen observer even as a child, and didn't notice signs of his mother having an affair. That suggests that it was just a temporary thing. The second was that Blythe didn't have children with her husband, in a time where people usually didn't deliberately limit their family to one child. Maybe John was sterile.
